Case Study: Osteoarthritis (OA) of the Knee

Knee osteoarthritis is a degenerative joint disease caused by the gradual breakdown of articular cartilage. It is commonly seen in elderly men and women and can be either primary (due to aging) or secondary (resulting from trauma or conditions like RA). Left untreated, it may progress to severe joint disability.

In OA, enzymes such as matrix metalloproteinases (MMPs) degrade cartilage, disrupting its natural balance. Despite the body’s attempt to compensate, cartilage elasticity and structure are lost, leading to stiffness, pain, and reduced mobility. OA is more than just age-related wear and tear; it’s a progressive disease requiring intervention.

Symptoms of Knee OA

  • Gradual onset of knee pain

  • Pain worsens with activity, climbing stairs, or after rest

  • Swelling and stiffness in the joint

  • Decreased ability to walk or perform daily tasks

  • Muscle atrophy, gait abnormalities, and joint deformity in advanced cases

Ayurvedic Management of OA Knee

Ayurveda offers a multi-pronged approach for managing osteoarthritis:

  • Lifestyle & Weight Management – Dietary changes and low-impact exercises to reduce joint load.

  • Herbal Medicines – Vata-pacifying herbs to lubricate joints, reduce inflammation, and strengthen bones and cartilage.

  • Shamana Chikitsa (Palliative Therapy) – Use of internal medications to manage symptoms.

  • Shodhana Chikitsa (Cleansing Therapy) – Panchakarma treatments to eliminate toxins and restore doshic balance.

  • Local Treatments – Includes medicated oil massages (Snehana), hot fomentation (Svedana), and knee braces for support.

Preventive Measures: Regular exercise, weight control, healthy diet, avoiding joint overuse, and timely treatment of injuries can help prevent OA or slow its progression.

Case Study: Frozen Shoulder (Adhesive Capsulitis)

Frozen shoulder, or periarthritis of the shoulder, is an inflammatory condition that restricts movement in the shoulder joint. It is most common between ages 40 and 60 and is more prevalent in women.

Symptoms & Stages

  • Dull, aching pain in the shoulder

  • Stiffness and restriction of shoulder movements

  • Radiating pain down the arm

  • Difficulty with daily activities like dressing or brushing hair

  • Sleep disturbances due to pain

The condition progresses through three stages:

  1. Freezing Stage (3–6 months) – Intense pain, especially at night, with a gradual loss of motion.

  2. Frozen Stage (3–18 months) – Pain subsides, but joint stiffness becomes severe.

  3. Thawing Stage (6 months–2 years) – Gradual return of mobility.

Ayurvedic Treatment for Frozen Shoulder

In Ayurveda, this is seen as a Vata disorder, where joint movement is obstructed due to internal or external factors (Nija or Aaganthu). Treatment aims to unblock the obstruction and restore mobility:

  • Internal Medications (Oushadha) – Strengthen nerves and muscles, reduce inflammation, and relieve pain without side effects.

  • External Therapies (Sthanika Chikitsa) – Improve circulation, relax muscles, reduce stiffness, and rejuvenate the joint.

  • Panchakarma Therapies – Include:

    • Nasyam – Nasal cleansing therapy for neurological balance

    • Snehapanam – Internal oleation to pacify Vata

    • Swedanam – Medicated steam for muscle relaxation

    • Mridu Shodhanam – Gentle detoxification for chronic cases

  • Physiotherapy Support – Assists in the recovery of range of motion and muscle strength
